The purpose of the review was to assess the planning, operation and delivery of matchday safety, security and medical provision associated with the tie, with a view to determining lessons learnt.
The scope of the review sought to:
Establish the facts surrounding what happened during the day of the Match, both within Ibrox Stadium and the surrounding area, including examining events and decisions made in advance of, during and after the Match.
Examine the planning and preparedness of all stakeholders involved in the planning, operation and delivery of matchday safety, security and medical provision associated with the Match.
Assess the roles and responsibilities and the adequacy of the responses to events, both within Ibrox Stadium and the surrounding area, on the day of the Match.
Examine all aspects of planning and arrangements for matchday safety, security and medical provision for the Match, identify any areas of weakness and recommend changes.
Identify any lessons learnt and to make recommendations to ensure there is no repeat of similar scenes at other fixtures with a similar risk profile.

“On behalf of the board I would like to thank the Review Lead, Mark Blackbourne, for producing such a comprehensive independent review and accompanying recommendations, which we will consider in full and seek to implement stated recommendations in conjunction with relevant stakeholders.
“Whilst this is now a live matter for an independent Judicial Panel to consider, the Scottish FA wishes to reiterate that the scenes witnessed during the cup tie, and subsequently investigated by our Review Lead, should have no place in our game.
“We will work together with the SPFL, clubs, Police Scotland and Scottish Government to ensure no repeat of the events that tarnished the image and reputation of the game. We will also seek to reassure the majority of fans who attend matches responsibly that showpiece occasions such as these should be enjoyed safely in future.”
